SERVICE - SCOPE

The Client understands that the Dietitian provides dietary, nutrition and wellness information, advice, education, coaching, support and counselling Services to enhance the Client’s knowledge of health as it relates to foods, dietary supplements as well as behaviours associated with eating, fitness, and wellness. The Services offered are at all times restricted to consultation on nutrition intended to build wellness and facilitate normal physical health and well-being. The Dietitian does not dispense medical advice nor diagnose diseases. These Services are not a substitute for treatment, or care of disease by the Client’s health care team, defined as physician, registered nurse and other credentialed/qualified members. 

 

SERVICE - APPOINTMENTS, DELIVERY, CANCELLATIONS, REFUNDS

SERVICE DELIVERY

• Services are delivered on an appointment basis. Drop-in services are not offered.

• All Service related documents including invoices are provided via email (paper copies on request) within 24 to 48 hours of appointment completion.

• Business hours for APPOINTMENT delivery: (M-F: 2 pm to 8 pm). Office remains closed outside business hours, on all statutory holidays and from 24th December to 1st January.

• Service delivery will be provided as opted by you and outlined in your client service consent agreement & contract.

• Log-in access to your online resources is active until the service completion date ( 7 days after the most recent appointment has been completed with no further appointments requested or scheduled)

 

SKILL REQUIREMENTS to participate in the plan (Services) –

• Ability to understand, read, write and communicate in English 

• Computer skills to ​access and operate email and internet, open files attached to emails, access forms by clicking links on email, fill out data and submit forms on website. 

 

REIMBURSEMENTS:

• It is advised that you confirm your extended coverage regarding 1) type 2) nature of services covered, 3) coverage amount, period and all other applicable details prior to accessing services and verify it before providing us with these details.

 

REFUNDS

• FULL Refund is available if you choose to cancel a 'booked and paid' appointment PRIOR to Service delivery. A minimum of 24-hour notice is required for all cancellations with full refunds.

• Services already delivered are non-refundable.

 

CANCELLATIONS

A minimum of 24-hour notice is required for all appointment cancellations. Notice can be provided over email, voicemail, text or SMS.A 25$ cancellation fee is applicable to all notices under 24 hours or for no-show appointments.

• You acknowledge that Bijal Thoda is a Registered Dietitian (R. D) who is acting in her capacity as a Nutrition and Lifestyle consultant for Posaitive Enterprises to provide Services that include information and education about diet and nutrition for health and weight management. We provide ongoing support to empower you and to help you take action to improve your lifestyle practices.

 

• Our Services are NOT a substitute and DO NOT replace any type of medical, surgical, pharmaceutical or other types of care treatment or professional advice that you may be currently receiving or may be scheduled to receive from any other care provider.

  • You should consult your doctor, if you are suffering from conditions requiring medical attention, or experience symptoms that concern them, at any point during the course of our Services. 

  • You should not cease conventional treatment or medication for any reason without consulting your health care provider.

  • You should always discuss any fitness regime, dietary changes or potential dietary supplements use with your doctor, and health care team.

  • We only provide recommendations to help incorporate behaviour modification techniques required to optimize dietary and lifestyle outcomes. Our Services are NOT intended to provide in-depth psychological counselling or family therapy, which is considered beyond the scope of our practice).

  • Services offered to the you are tailored to the your individual health profile and is intended solely for your individual use.

 

• NO GUARANTEES: Any information on this website is only cited as an example and by no means imply any guarantee of any type what so ever. Our Services provide you with the tools, techniques, information and support to empower you to take responsibility for your choices, actions and decisions to work towards the results but NOT GUARANTEE any results.
